De Soto Multi-Service Center:
Community Service Management

Similar to traditional case management, community services management assists individuals and families experiencing difficulty meeting or addressing their social and economic needs. Coordination of multiple services often helps to assist an individual or family move toward self-sufficiency.

Who may participate?

Any family or individual experiencing difficulty meeting or addressing basic needs may participate.

What multiple services might a family need?

Here is an example of a family situation that may benefit from community services management.

S. is a single mother of two children. She was recently diagnosed with multiple sclerosis and has been unable to work. The Community Social Services Manager (CSSS) was able to connect her with the MS Society for assistance with her medications and equipment. An application for disability through the Social Security Administration was initiated, and an application for medical, food, and cash assistance was completed and turned into SRS. The Multi-Service Center assisted with food, a utility payment, and a portion of rent. The CSSS contacted the school and is working with the school Social Worker to ensure that the children are receiving appropriate services to help them cope with their mother’s illness. The CSSS is helping the mother make some long-range plans regarding care for the children. Regular, on-going support is an important component of community services management.

Is there a fee for this program?

There is no cost for this program, but donations are always appreciated.
