What is geriatric care management?

Geriatric care management focuses on assisting individuals age 60 and over to live independently in the community. Services include assessment of individual needs, development of an individualized plan of care and locating and establishing services. After services have been established, the care manager monitors services and annually reassesses the individual to identify changes in needs and to assist with any needed changes in service.

When is geriatric care management needed?

Older individuals who are living alone, with limited income, and who are beginning to experience difficulty with managing every day tasks may benefit from geriatric care management. A geriatric care manager can address a single issue or a complex situation requiring coordination of many services.

How do I access geriatric care management services?

Contact an aging information specialist at HSA-aginginformation@jocogov.org.

How often will the care manager visit after the initial assessment?

The care manager will periodically make contact by telephone and will visit in person at least every 6 months to ensure that the care plan is still meeting the individual’s needs.

What is the cost of geriatric care management services?

There is no charge for geriatric care management. Geriatric care management services are funded by federal, state and local government. Donations are accepted
