Case management is assisting persons with a disability in assessing needs, finding available resources; and connecting them with those resources so they can live as independently as possible.

What services do you typically help individuals with a disability obtain?

We typically assist with applications for Social Security disability benefits, supplemental security income, food stamps, Medicaid, and other state, federal, and local services.

We assist hundreds of individuals with the paperwork required to apply for these services and also are able to act as an advocate for you at an appointment with the social service provider.

Is there a cost for case management services?

There is no cost for individuals with a disability.

I need an apartment. Will you find one for me?

We do not assist with finding apartments. We can assist by providing lists of Section 8 and HUD properties.

I need a job. Will you find one for me?

We do not get involved with job placement. We can provide information on job placement agencies and help you apply for vocational rehabilitation services.

I need a ride. Can you transport me?

We do not provide transportation, but can help you find public, private, and volunteer transportation.

I need help making my home or apartment more accessible. Can you assist with this?

Yes, the case management staff can help you apply for county funds to make your home or apartment more accessible.
